What documentation will Co-operative Bank ask me to provide?

You will be required to complete the Bereavement Instruction Form, which can either be sent by post, or delivered to your local branch.

As part of this form, you will be required to fill out the following information:

Details of the Deceased:

  • Name
  • Address
  • Date of Birth
  • Date of Death

Account Details:

  • Account Number
  • Sort Code
  • Credit Card Details
  • Loan Account

Details of Personal Representative(s):

  • Grant of Probate / Letters of Administration / Certificate of Confirmation
  • Name of 1st Representative
  • Date of Birth
  • Address
  • Telephone Number
  • Relationship to the Deceased
  • Nationality
  • Name of 2nd Representative
  • Date of Birth
  • Address
  • Telephone Number
  • Relationship to the Deceased
  • Nationality

Details of the account you would like the funds to be paid to:

  • Payee Name
  • Bank Name
  • Account Number
  • Sort Code

What happens next?

After you have notified Co-Operative Bank, you should be sent a letter confirming in writing what further steps may need to take place, including any additional documentation they may need. 

They may also apply the following steps:

  • Freeze accounts until account closure
  • Cancel any standing orders and/or direct debits
