How do you notify Aviva Insurance of a Death?

Aviva Insurance allows you to notify them of a bereavement in two ways. You can either phone them on 0800 092 5367, or you can complete a claim form and send it to the following address:

What information does Aviva Insurance need for a Death Notification?

Aviva Insurance requires you to have the original death certificate as they do not accept a copy. You are also required to obtain copies of any grant of legal representation. 

What Information is needed for the Online Form?

  • Policy Number
  • Type of Claim – A Death Claim
  • Name of the Deceased
  • Address of the Deceased
  • Date of Birth of the Deceased
  • Date of Death of the Deceased
  • Cause of Death (stated on the certificate)
  • And lastly, your personal details.

Once you have notified Aviva Insurance of a death, they will contact you either by telephone or email if additional information is required.
