How do you notify Aviva Insurance of a Death?
Aviva Insurance allows you to notify them of a bereavement in two ways. You can either phone them on 0800 092 5367, or you can complete a claim form and send it to the following address:

What information does Aviva Insurance need for a Death Notification?
Aviva Insurance requires you to have the original death certificate as they do not accept a copy. You are also required to obtain copies of any grant of legal representation.Â
What Information is needed for the Online Form?
- Policy Number
- Type of Claim – A Death Claim
- Name of the Deceased
- Address of the Deceased
- Date of Birth of the Deceased
- Date of Death of the Deceased
- Cause of Death (stated on the certificate)
- And lastly, your personal details.
